Complete NC-4/NC-4 EZ withholding forms by Dec. 20

The Department of Revenue requires all employers to request new Employee’s Withholding Allowance Certificates (Form NC-4 or NC-4 EZ) from employees as a result of N.C. House Bill, effective Jan. 1.

In the event a new form is not submitted, the employer must set the tax withholding to a status of “single” with no allowances, which will maximize the amount of taxes withheld.

The University’s Payroll Office is requesting employees to submit a new form by Friday, Dec. 20, 2013, to ensure all forms are processed prior to the first payroll in 2014. Employees can submit either form; guidance is on the Web (see page 2). Forms are available online, too (NC-4, NC-4 EZ).

Completed forms should be sent via campus mail to the Payroll Office in Reese Building or hand-delivered. Do not email the form as it asks for Social Security Numbers.
